الرواكتان * انتهيت من العلاج بالرواكتان هل أحتاج أن استخدم مراهم او كريمات مع العلم أن بشرتي فيها بعض الآثار والندبات البسيطة، وهل مازالت الشمس تضر ببشرتي؟
- يعتبر الرواكتان من الادوية الفعالة لعلاج حب الشباب، وينصح بعد استخدام الرواكتان استعمال كريمات التقشير الخفيفية مثل كريمات فيتامين (A ) وكريمات حامض الفواكة التي تساعد على المحافظة على النتائج التي حصلت من دواء الرواكتان. كذلك قد تساعد على التخفيف من الندبات البسيطة ولكن على المدى البعيد. أما بالنسبة للتصبغات فينصح باستخدام كريمات التبيض مثل الهيدروكينون وحامض الازيللك أسيد.
أما بالنسبة للندبات البسيطة فيمكن التخفيف منها بواسطة ليزر الصنفرة غير التقليدية (الفراكشنال ليزر) وذلك بعد الانتهاء من استعمال دواء الرواكتان لفترة 6أشهر، ويحتاج المريض إلى حوالي 5 جلسات للحصول إلى النتائج المرجوة وفي كل الحالات ينصح بالاستمرار باستخدام واقيات الشمس.
الحمل والجلد
* أنا حامل في الشهر الثاني ماهي التغيرات المتوقعة على الجلد خلال الحمل .
- أثناء فترة الحمل تحدث الكثير من التغيرات في الجسم ومنها التغيرات الجلدية التي يمكن تقسيمها الى:
التغيرات الفسيولوجية
1- تغيرات صبغية:
تلاحظ جميع النساء خصوصا ذوات البشرة السمراء زيادة في التصبغ خلال الحمل في مناطق مثل الابط، وحلمات الثدين والفخذين. اضافة إلى زيادة اغمقاق لون الندب والشامات، يعود فرط التصبغ إلى ازدياد مستويات البروجسترون والاستروجين وهما هرمونان من شأنهما تحفيز إنتاج مادة الميلانين في الجلد .
وكذلك الكلف (melasma) والذي يصيب الوجه كما ويعرف ايضا بقناع الحمل ويظهر على شكل بقع سمراء إلى بنية اللون مبقعة ومنتشرة بشكل متناظر وأكثر اماكن ظهوره الجبهة والذقن.
يصيب الكلف من 50- 70% من النساء الحوامل و 5- 34% من المستخدمات لاقراص منع الحمل. كما ان اشعة الشمس ايضا وهي من العوامل المعروفة المسببة للكلف من شأنها ايضا ان تزيد من فرط التصبغ. تختفي عادة بقع الكلف بعد الولادة بسنة.
2- التشققات الجلدية:
من أكثر التغيرات حدوثا، تصيب 90% من الحوامل وعادة ما تظهر الاصابة بها في أواخر الثلث الثاني من الحمل حيث تظهر على شكل خطوط زهرية إلى بنفسجية اللون ومن اكثر مناطق الجسم اصابة بها البطن، الصدر، الارداف، الاذرع، الافخاذ. يجهل سبب الاصابة بخطوط الحمل رغم انه يبدو بأن الاستعداد الوراثي والهرمونات واكتساب الوزن خلال الحمل هي مسببات تكونها. لايوجد علاج لخطوط الحمل لكنها تميل لتصبح تقريبا بلون الجلد بعد الولادة دون اختفاء مظهرها.
3- تغيرات بالأوعية الدموية:
وتحدث هذه التغيرات بسبب زيادة هرمون الاستروجين ومنها زيادة في الشعيرات الدموية وتظهر عند 70% من الحوامل.
كذلك احمرار باطن الكفين وهو مرض يصيب ثلثي النساء البيض وثلث السمراوات وعادة يتلاشى بعد الولادة بأسبوع او اسبوعين.
اما الاصابة بالدوالي varicosities فهي امر شائع يحدث عند 40% من الحوامل وتعزى الاصابة به لازدياد الضغط على اوردة الحوض والفخذ ويمكن ان يتفاقم وضع الاصابة بالدوالي ليتحول إلى الاصابة بتجلط عند 10% من الحوامل.
ايضا من التغيرات الوعائية التهاب اللثة في الحمل pregnancy حيث يحدث ألم باللثة مع نزيف ويظهر عادة في الاشهر الاخيرة من الحمل.
4- تغيرات على الشعر:
تتغير خلال الحمل المراحل الدورية الطبيعية لنمو الشعر ونتيجة لهذا فإن عدد جريبات الشعر في مرحلة النمو يزداد في حين ان عدد الجريبات في مرحلة الخمول والتساقط يقل ويسبب هذه التغيرات التي تحدث خلال الثلث الثاني والثالث من الحمل تلاحظ معظم النساء الحوامل ازدياد كثافة شعر فروة الرأس اما بعد الولادة فإن الوضع ينقلب وتكون هناك بقايا من الجريبات في مرحلة الخمول ويزداد تساقط الشعر ابتداء من 4- 20 أسبوعا بعد الولادة.
كذلك يكون هناك زيادة في نمو الشعر بالجسم وهي زيادة مؤقتة تعود إلى المعدل الطبيعي بعد 6 أشهر بعد الولادة.
